1. Must be a graduate of a bachelor’s degree.
2. Submit the following to the Office of the Dean from 2:00 PM to 7:00 PM, Monday to Friday:
a. Application Letter addressed to:

Sudapin, Kidapawan City
b. Completed Application Form
c. Two (2) Passport size pictures with nameplate & white background
d. Transcript of Records
e. PSA Birth Certificate
f. Good Moral Certificate/Honorable Dismissal
g. One (1) long brown envelope
*For TOR & Birth Certificate – Legible copies will be accepted, in which case, a notarized undertaking must be executed that a certified copy will be submitted.
**For Good Moral Certificate – In lieu of the original or certified copy, an undertaking will suffice, but must be attested that the original or certified copy will be submitted.
