Procedures for Complete Withdrawal of Enrollment PDF Print E-mail

Processing of this request is done when an entering freshmen or transferee wishes to withdraw or cancel his/her enrollment from the University after his payment, or after undergoing the final screening process. This request has to be made within ten (10) days after the last day of enrollment.

STEP PROCEDURE WHAT TO PREPARE, BRING, ACCOMPLISH AND SUBMIT WHAT TO DO

APPROVAL
  • Certificate of Registration
  • Accomplished Dropping Form
  • Withdrawal Form
  1. Go to your respective college/unit and ask for Withdrawal form.
  2. Accomplish the form.
  3. Go to the Admission and Records Office (ARO) and secure Dropping Form.
  4. Accomplish the Dropping Form
  5. Credentials/Documents you have submitted during Final screening process will be returned to you.

CLAIM FOR REFUND(please refer to USM Academic policy regarding Refund claims)
  • Copy of the accomplished Withdrawal and Dropping forms
  • Refund form from the Admission and Records Office and accomplish the same
  1. Present copy of accomplished Withdrawal and Dropping forms
  2. Get Refund form from the Admission and Records Office and accomplish the same
  3. Wait for their advice as to when you can get your refund.
